The Guardianship Service

 

When a beloved family member loses their ability to act responsibly for themselves, the strain on those around them that care the most can become overwhelming.   Having a caring professional available in the role of the legal guardian can help ease the family tension and help the family member’s roles get back into balance. 

For individuals who have been identified by a physician to be no longer able to manage their personal affairs, Easternview LLC provides staff that can be appointed as legal guardian.  We are more than simply good advocates for their care.   We want to get to know who they have been in their lives to tailor our time with them to honor who they are and what they value.  

Guardianship services include:

 

·        Meeting regularly with the client, acting as an advocate and allowing them as much input as possible into decisions about their care.

·        Coordinating with a reputable consultant, if placement is necessary, who will locate a facility that incorporates input and is respectful of the client and family’s stated needs and desires.

·        Attending treatment meetings and serving as the contact person for the facility treatment staff.

·        Providing regular reports and updates to the court and family members and to be available to address any care-related concerns that may arise.

 

This service is charged at a rate of $65 per hour. *

*This service does not include management of the person’s financial affairs.

We recommend contacting your bank’s trust officer or your family attorney for referrals.

The Good Neighbor Drop-In Service

 

For individuals who are not in need of guardianship services, Easternview LLC offers a Drop-In Service. This service is geared for the frail, older adult who may need some support and oversight to better assure they can continue to live safely in the least restrictive, most familiar setting, as long as possible.  Routine visits are provided as frequently as necessary for:

 

·        Socialization and evaluation of the person’s current and on going needs.

·        Obtaining, procuring and supervising necessary auxiliary service referrals, such as linking a client with Meals-On-Wheels, arranging transportation to medical appointments, or connecting them with Senior Day Programs and other activities or services.

·        Keeping long distance loved ones regularly informed.

·        This service is also available on a temporary basis, such as during and following your loved one’s hospital stay when long distance family members may not be available.

 

This service is charged at rate of $40 per hour. *

 

 

Eastenrview LLC Staff

 

 

SALLY L. WILLIAMS, LMSW – Professional experience includes over ten years in community social work practice working extensively with persons in need of court intervention to assure their continued safety and well being.   A seasoned professional, in supporting clinical staff with sorting out complex treatment and ethical issues in emergency situations.

Memberships include the National Guardianship Association and the National Association of Social Workers. Professional credentials include Licensure as Masters Level Social Worker (LMSW).  Fully insured.
